I don't know how great a poem you need, but the site that is provided below as the source gives the words of a large number of simple poems (jingles really) about change.

if you mean written perspective i am currently reading Pedro Páramo which has many shifts in perspective from that of Juan Preciado to that of his father Pedro to that of people now dead, etc. however, these perspective shifts makes it difficult to read. a book that i really enjoyed, and finished reading a little while ago, would relate to change in perspective in characters and is entitled The Cave. The Cave relates to Plato's allegory of the cave and many of the characters go through perspective changes due to either a crisis or a revelation.
