This may be somewhat of a Trick Question.
If the Goalie is not In the crease, The Referee Cannot Blow the Whistle.
The Goaltender must be in the Crease until the Player Touches the PUCK. It is not based on the Whistle.
If the Goalie is out of the Crease Before the player touches the Puck, then the attempt is allowed to continue and the player will get a second attempt if it is not a Goal.
Also, Shoot-Out and Penalty Shot rules are the same.

If the goalie leaves the crease before the whistle in a shootout or penalty shot, play resumes until the end of the shot. if no goal is scored the shot is taken over. if a goal is scored the goal stands.

If the goalie is out of his crease before the ref blows the whistle he shouldn't be blowing the whistle but skating over to the goalie and telling him to get back in his crease. With two referees, it's hard for a goalie to get away with being out of his crease at the start. If somehow it was the case they would have to redo the shot.

if the ref missed the whistle but is in position to blow the whistle then no goal. But i think if the ref missed the whistle but there was no goal i think its just no goal. i don't think there's any retake or something like that in the shootout unless there's a problem with the net or puck or stick.

Good question, I'm not sure, but I don't think it makes a difference since most shootout situations are usually clearcut in or out situations.

My dad played professional hockey for all his life so trust me on this one. The shooter gets the point and there is no shot taken automatically, plus i was a goalie for three years! TRUST ME!
