I have PCOS too. I am on my third pregnancy but it has not been easy, this is my first pregnancy to go full term due to complications not related to PCOS. My first two were spontaneous this one was achieved with the help of metformin and clomid. With your advanced maternal age I would go to your Dr immediately and get a re feral to a reproductive endocrinologist. You should also have a glucose tolerance test done to see if you are insulin resistant or diabetic. I am insulin resistant making conception harder. You should also get your progesterone levels checked, mine are low and I need to supplement or I'm not regular at all. Some things that you can do with out a Dr are to follow a low carbohydrate diet, exercise, take glyco-nutrients, use a wild yam cream to supplement your progesterone and get your cycle regular, exercise, and chart your cycles to see if you are ovulating (not everyone with PCOS has a lack of ovulation)
