5.(a) passive ('was broken'- if it said, "My cousin broke the bicycle," then that would be active.)
(b) passive ('was eaten'- if it said, "The mice ate my grandmother's cane," then that would be active.)

1.. Depends if you are in the UK or US--US would be wins, thinking of the team as an entity--UK, would be win.
2. want--sisters is the closest subject.
3. enjoy--both of them comprise the subject
4. whom--the object of selected
5. Passive--both of them, the subject was acted upon, didn't do the action itself.
