Aside from its obvious geographic reference, it is loaded with symbolism. Cal is a hermaphrodite--half male and half female. Essentially, he's in the middle of the sexes. This idea of middlism runs through the books. The family lives in suburbia--halfway between country and city. At times, Cal appears to be bisexual, having a crush on the Obscure Object but sleeping with her brother. If you read it closely enough, there are tons of references that will let the title make a lot of sense.
