Well, you would be wrong...the thing is that it depends on what kind of services you are looking for...and the fact that mafia refers to organized crime groups in Sicily (and is now applied more broadly) anyways...Italian mafia is has been around for some time so they are pretty well organized know their business. But then you have the Russians, the Yekuza, the Albanians, the Serbians etc...
I would say that the most terrifying to deal with are the ALBANIANS!!! Stay away from them...no offense guys...but they are the cruelest, most ruthless SOBs there are.
Russians are a close second but they are a bit dumb.
If I had to pick the greatest of all I would go with the Yakuza. They are ruthless enough, organized and, well, Japanese!

Technically, it would be Sicily. Italy didn't actually have a Mafia; but since at the time the Mafia was very big Sicily was considered a part of Italy, it was just called the Italian mafia.

Yes Italian is classic country,but I think Russian mafia is the biggest in the world.After Russia is Bulgaria brother of Russia.Never mint that now Bulgaria is EU country and Friend with USA.In every moment they are ready to turn back to Russia.
But for moment not existing biggest mafia of Russian.
