it depends on the format you are using, if you are using MLA then your cover page should be vertically centered with your topic
your name, the name of the teacher, the class, the class period, and the date. if you are using APA then you need all of the above but you need the name of the school in between your name and the name of your instuctor

It really depends on what format your are writing in.....most research papers would be done in APA format and a common cover sheet would be something like this:
