Water turning the wheel turns energy into whatever output you decide to use.

A water wheel does not really "create" energy. It transfers energy from the moving water to the rotation of the wheel. The water pushes part of the wheel, making it spin; but the energy causing the wheel to spin is "removed" from the water, so the water would be going slower than it would otherwise.

The point is you take energy from the river to rotate the wheel, which rotates a shaft, which can be attached to anything you want to do "useful work."

You can't. Not by Itself. It is just a method of taking energy from moving water and turning the wheel to transfer the energy to your need. The natural movement of the water contains the energy. @8-)

You can either connect it to a shaft, then to a generator to make electrical energy,

OR

you can connecto it to a shaft, and to some pulleys and belts to turn machinery mechanically.

OR

you could connect it to a shaft, and have it turn a grinding wheel, to produce heat energy.
