The condom could be old (they actually do expire and then dry out), wearing a condom too small for your junk (this can happen too), female is really dry during intercourse causing friction

Yes, it happened only once!!! i think that happend because we did not put out the air before putting it on and also becauseit was a fast and hard penetration:)))

Not really. But I once met a guy who refused to wear one because he said he didn't know how to use it (he was 36), so I dumped him. Good luck.
