It means to tell the truth

"I know what you did last night, come clean!"

To 'come clean' is to finally admit the truth (that's under the layer of muck that represents lies or cover-ups).

For example "she couldn't live with herself any longer, she had to come clean to her boyfriend about cheating on him"

or "the situation had erupted into chaos. The only way to solve it was for someone to come clean about who started it"
