"The trains collided by a river."

In this phrase the word "by" is being used to describe the fact that the trains collided next to a river, not that it was caused as a result of the river. In all other examples, the word "by" is is directing us to who caused the actiion.

Perhaps because it's a few tourists. Usually a GROUP of tourists aren't attacked by a gang and if they are, usually individually....

And specifically 'tourists', maybe that's a point too

in regards to the post above, earthquakes can be perceived by some as having being self-inflicted. i.e., people were bad --> bad deeds --> God angry --> God's wrath

all the others were man made or inflicted, so to say, the earthquake was a natural of act of god disaster.
