Denial of himself.
A man can deny his weakness and fall by having that exploited. He can deny his strength to the same. He can deny his wisdom and his ignorance. Worst of all, he can do all, and so much more, at the same time.
Success comes in knowing oneself: abilities, strengths, weaknesses, etc... It comes in learning that, no matter how good you are at anything there is always someone better. It comes in learning from that someone better rather than seeing him or her as a rival.
Man's downfall is in failing to realize how much greater the world is than he.
The same is true of women. We, at least, have that extra leg on that one chromosome. It does help ;-)

Under any reason. Some people are not self-assured enough and they're permanently afraid to make a mistake. Somebody is not able to take responsibilities and doesn't want to become an adult. Some moment he or she loses the boundary between real life and the dolls he is used to play with. They still try to play their game even if somebody happen to treat them seriously. Somebody has not correct opinion about himself. It's a problem because when his own opinion of himself and other people's opinions of him don't co-inside he also makes a lot of mistakes.

Bad decision making without weighing the consequences. Sometimes the smallest decision can cause a ripple in your life that is magnified many times. At the other end of the spectrum is the fear of taking chances. Both of these can lead to self destruction.

Also, on a deeper level, his downfall may be his failure to believe in God for no man can stand up without God.

It depends on the man...but I would say that in general we fail because we don't love ourselves enough. If you don't love yourself, you won't believe in yourself & will not succeed. If you don't love yourself then neither will you feel love from or for others & you will not find happiness. With love, there is nothing you can't do. Without love, there is nothing you can do.
