It is, both in the sense that the one you use it to is likely to be offended, and that it refers to an old and not very enlightened way of refering to people with mental handicaps.

Still, I rather fancy it as an insult (to someone who does not, obviously, suffer from any mental handicaps). It has a quaint, old-fashioned feel to it, and is just a little too ridiculous to be taken seriously.
