I heard he suffered a gruesome accident because Bale's character was not paying attention. What was the gruesome accident?

specifically, the friend was working on some sort of drill press, or something with a spinning piece, which operates at very hig RPMs. then Bale accidentally hits the on switch due to his insomnia and paranoia. the friends hand is first punctured and then is violently torn off his arm. the hand is skewered on the piece and continues to spin before they turn the machine off.

Christian Bale's character was looking up at the figure of someone in place of someone else and it so startled him that he backed into the on/off switch for the machine his friend was adjusting, and it turned the machine on, which in turn caught his friend's shirt, and caused his hand to be amputated before they could get it out.
As for the person Christian Bale's character saw...that was a phantom? One could speculate that it was Bale's character's conscience. But not many people were who they were supposed to be, all according to the past event that changed Bale's character.

If I remember correctly he had his hand mutilated in some machinery.
