It's been forever since I read this, but I'll give it a shot.

I would say the protagonist is the narrator - he is the leading charcter, even if he isn't a good guy.
pro·tag·o·nist - [proh-tag-uh-nist –noun 1. the leading character, hero, or heroine of a drama or other literary work.

Then I would say the antagonist is the dead man who first of all drove the main character to commit murder and then, once dead, drove him insane (or something like that) with the beat of his heart. I mean the nerve of that guy! Hah!

The night watchmen seem sort of like scenery to me - an audience for the story. I read this to my 7th graders one year with a heartbeat in the background. Pretty cool.

Hm...I just read the other comment. Maybe I am out there on this one. Still, hope it all helps.

The murderer is antagonist. The old man is the protagonist. the story is just told from the antagonist's point of view. And the officers are just plot points to round up the story. At least that's what I think, but I haven't read it in a long time.
