I know that similes use "like" or "as", but what about "as though"?

Is this considered a simile.

My mother listened gravely to the music, as though synchronizing her heartbeat with it.

no. Mainly because its still using the word "as".

it also couldn't be "as a" either.

I agree- I think it's a simile. In fact, if you're nit-picky, you may argue that it in fact does use "as"...

i would say yes. it may not be a traditional simile, but it is a comparison that is not a metaphor.
