I want to put a form on my myspace page for people to fill out. I can't find any codes that will help me to do so. All I want is a simple form.

Age
Comments
Submit button
You know the usual stuff.
Please can somebody help? I have been trying to get this together for 2 weeks

place the sample piece of code in a html file, open the file in a browser it will display a sample form. Then you just go through the code you will understand how is it done.

but the best thing is you buy a book of html or download some tutorial from net and go through them. Form is not such a tough ask.
