Go to the courthouse and talk to the clerk of courts. Laws differ from state to state and the clerk of courts can help you with the proper paperwork and fees.

If you havent actually filed for divorce you can have that placed in the divorce decree...that way it is done and over with then all you need to do is bring a copy of the decree to the drivers licence station and the social security office to get issued new cards...otherwise you have to go to court and tell them why you want your name changed etc....it will cost you like 100.00 I think...then you can go and get your new cards once you have a letter from the court saying that you legally changed your name!

if your still married at the last name request in your paperwork if not go to the court house or find an attorney to file last name change really easy to do. can be spendy. each state has there own to do stuff

are you still married? either way, i think its just a matter of changing your social security #.... once thats changed, everything else can be too, with just ur phone call... call ur local social security office and ask someone how you would go about doind that....
