What should a reflection include?

I had to do a project for health class and now I have to do a reflection but I dont know what a good reflection has to include.

A relection basically asks you to talk about your project.

1. What did I think about my project?
2. Did I do a good/poor job?
3. If I had to do it over what would I change?
4. If I had to give someone advice about completing this project what would I say?
