For the same reason other animals have twins:
1) a fertilized embryo split sometime during development
2) two embryos were fertilized at the same time

In other animals, twinning is not uncommon. But since foals' legs grow so much before they're born, it's more of a big deal when a mare has twins. Also, the uterus in horses is shaped slightly differently from that of other animals...there's not really room for more than one at a time. Any mare who you suspect of having twins should be under the care of a vet, as this can cause problems at birth (some sort of presentation inconvenient for birth, one's umbilical cord wrapped around the other, congenital defects, or even incomplete development) She may deliver them okay on her own, but a sonogram is usually a good idea to view any potential problems.
