I have PhD in financial economics, and they only cared about my college GPA.
If you want to be a medical doctor, then you will need to be a premed major (in chemistry or biology). Most of the premed programs care mostly about your math and science grades, but you still need a decent overall gpa to get in (at least a B average). So, you have good chances of getting into a decent pre-med program. Medical programs (in which you enroll after completing college) and PhD programs only care about your college grades. If you want to go for a PhD, make sure you get some research experience, even if it means waking up at 5 am to work in a lab.
If you are going for MD, try to get a job in a clinic, even as an administrator, or volunteer for a free clinic- medical schools love that.
As for schools in the midwest- Northwestern is very good, University of Chicago is also good, Ohio State has several good programs, Washington University at St. Louis is good, and Penn State and U. Penn have excellent research programs.
There are also plenty of smaller schools which have individual strong programs, and aren't too expensive.

It would depend on the criteria of each university. If you don't make the cut, I would imagine that it was your english score holding you back. I might suggest a bridging course in english to raise your grades.

However, your maths and science scores may pull you over the line. Good luck

I hold a doctorate, but I don't think it's required to answer your question.

A doctorate program is a grad school. They just care what your grades were in college and above. They don't ask or care how you did in high school.
