The worst game that I've ever played would have to be Dark Summit for the XBX. After getting used to the speed of SSX, Dark Summit was like a turtle. The game was extremely slow for a downhill snowboarding game, and the buttons didn't correspond to the moves either. You had to enter the button sequence first before she would do the moves. How lame is that?
As for worst video game ever made, it's basically a toss up between Superman for the N64, Fantastic Four for the PSX, Mortal Kombat Mythologies: Sub-Zero for the N64/PSX, and Aquaman for various consoles.

I forgot what it was called, but it's this really dumb truck racing game that wasn't even completed; you could drive thru wall and stuff.
