There's a double entendre to Hunter "Patch" Adams' nickname.
1) It's a clownish name, particularly for perhaps a hobo clown, and Adams was often seen wearing pieces of clown costumes and gear when seeing his patients.
2) As a physician, it's his job to repair or "patch" up any injuries or illnesses the patient might have.

OOOOOH MAN!! what an EXCELLENT movie!! and being a true story made it even better! robin williams ROCKED in that movie!! he was called 'patch' because he 'fixed' everyones problems. actually he says that in a confrontation with the 'boss' of the hospital in the movie. i gotta rent that one again! i would love to meet the REAL patch adams, but i think he's deceased, isn't he?

He got the nickname while in the mental hospital. Don't remember names or anything, but he was in a room talking to an older man about something. The older man's paper cup was leaking. Patch picked up the cup and fixed it so it wouldn't leak. The old guy then called him Patch and he stuck with it.

In the beginning of the movie when Patch was at a mental hospital, he patched up a hole in a cup that belonged to one fo the patients. The patient was very thankful and called him "Patch."
