In your Secretary of State website -- they have the instructions and all forms you need

However, you can hire a lawyer to set up your LLC for you. Or use one of the incorporation services on the Web

I just set up an LLC and a lawyer is a really big help. It sucks to have to pay for one, but they really do help. So, I'd recommend using a lawyer.
