You cannot get a duplicate P45 - that would leave the system open to abuse! When you start your new job, tell your employer you don't have a P45 and ask them for a P46 - you can sign this and your employer will send it to the Tax Office who will trace your records and send your previous pay and tax details to your new employer. You may be on emergency code for a while, but you will receive a repayment of any tax overpaid.

Unfortunately you are unlikely to get another as this is an official tax document but your old employer should be able to either
a - copy their portion for you
or
b - offer you a statement of earnings covering the last tax year.
If you need it to start a new job, they should be able to offer you a P46 to sign. You will probably be emergency taxed initially but once the tax office get up to date you will receive this back. If the P45 was more than 6 weeks old it would probably have been invalid or queried anyway so you will need a P46 to complete.

You only get a P45 when you leave a job so if you're in work now, you will be given one if/when you leave that job. If you're not working but signing on, the DWP will give you one when you sign off to start work.
Why do you think you need one? More info would help.

You dont need a P45 to start a new employment. Your new employer can use a P46 to complete their records with the tax dept.

Try asking your previous employer for a copy or a new one. They should have a record of the figures that were previously sent to you.
Failing that contact your previous tax office explaining the situation.They will contact your new tax office with the information required to correct your paye.
At this moment in time you could fill out a P46 given to you from your employer then hand this to your tax office.
