will the procedure have to be put on my medical records? does anyone know how confidential this is?

Not, if you don't want them to know. You can give out your gp's name and address on the paperwork or you can leave that section blank.

some people want their cosmetic surgery to be a secret and even go abroad to get it done. they come home from a vacation looking refreshed, with no one from home knowing they underwent any surgery. even if you don't inform your gp, your plastic surgeon will require some tests to be done before you can have surgery, to make sure you are healthy enough to undergo the procedure you are considering. you can have the tests done with another doctor, in the country you will have your surgery in. just make sure you choose board certified doctors. medical records are confidential so you can keep your surgery a secret.

Yes, it has to be put in the medical records and the general practitoner has to know.

And HIPPA law of confidentiality protects the medical information, nobody can access to your medical records without you knowing and accepting it. Nobody will know unless you tell.
