I haven't cleaned it in a long time and now all the rungs have a lot of dust caked on them. I can barely run a finger between the rungs so what is the best way to clean it? Especially the back?

W D 40 sprayed on will clean anything without harming the surface.

Is it metal?
I would dust it as best as possible with a feather type duster. Then, and this might sound daft, pour a little olive oil onto a piece of kitchen paper & rub on if metal. Don't leave too much oil on it though, because when the towel rail heats up it'll get really hot.
It'll shine like a new penny!

For very hard to clean surfaces that are not glass, aluminum, varnished or painted I use Super Strength Mean Green.

Toothbrush in hot soapy water

cover a thin cloth in warm soapy water and try and clean it with that. it might get further than your fingers.
