Most MBA's do require at least two years of work experience, there are some unranked MBA's that do not require work experience. Top ranked MBA's like Harvard, Wharton and Stanford have an average of 5 years work experience. MBA's courses are not exactly harder than undergrad business courses, in fact some courses are exactly the same as undergrad business courses, but in an MBA class, you have a lot of discussion. That's why they want students from diverse work experiences to make livelier discussions.
PhD's don't really require work experience, cause it's not discussion based, they are more research based, and thus you need to be mathematically and empirically strong.

Unfortunately, no it isn't. I thoroughly believe that a person should work in industry for at least 5 years before going to school for an MBA but I guess I'm the only one. I have an MBA and didn't get it until after I had put in about 20 years in business working as a technician then as an electrical engineer. I have worked with many MBA's that graduated when they were 24 or 25 and really should be peeling potatoes for Simplott rather than sitting in a management position. Just my opinion. School gives you a basis for working but certainly doesn't replace industry experience. A PhD, you usually need work experiance. Good Luck

NO!
You can go straight from high school to college to MBA without ever working in any capacity. It is not a prerequisite for admission to an MBA program. However, it is to your advantage if you actually have some work experience. You'd have a better appreciation of textbook theories and real-life applications.
