Apollo 13 certainly went to the Moon but it didn't land due to mechanical problems on the way. When they went behind the Moon to get the slingshot effect for coming home, Lovell, Haise and Swaggert were further from the Earth than any human before or since.

Apollo 13 the third manned lunar-landing mission, part of the American Apollo program. It launched on April 11, 1970. Two days after the launch, the Apollo spacecraft was crippled by an explosion, causing the Service Module portion of the Apollo Command/Service Module to lose its oxygen and electrical power. The crew used the Lunar Module as a “lifeboat” in space. The command module systems remained functional, but were deactivated to preserve its capability to reenter Earth’s atmosphere upon return to the earth. The crew endured difficult conditions due to severe constraints on power, cabin heat, and potable water, but successfully returned to Earth.

Yes. They went around it and came back they couldn't land because of many problems they had on ship, on the way there. They were lucky to make it back alive!
I believe that there is was a movie made called Apollo 13, staring Ton Hanks.

Absolutely, the crew from the grassy knoll in Dallas tried to stop it but they couldn't. Even Jack Ruby and Castro couldn't prevent it.
