"Be very careful" would be a great start. Otherwise tell her it is wrong and be prepared for your friendship to end. Since she already knows it is wrong and wants to do it anyway is there anything else you can do? If you tell the parents, the two of them will just say it's not true and your friendship will be over anyway.
You can't stop people from doing things that are not right, the real question is why are you still friends with this person?

I would be there for her, but tell her that if her parents found out they would be heartbroken, really ruin a family. what about pregnancy, that would really confuse a kid. It's a tough situation because you don't want to offend her but if it was me, I would tell her straight up that what she is doing is wrong. They are supposed to be a family and they will probably never be a family because what happens when they get older and get married, if their spouses knew about it they wouldn't be comfortable. She's probably not going to listen but she would at least know how you feel.

wow, what CAN you say. She is going to do what she wants weather you say something or not, if you do however say something, she might listen, but more than likely, she just wont talk to you about it anymore. It's weird, but as long as shes consenting to it, its her business. At least they are not really related.

I kid you not an aqautaince of mine was in this same type of a relationship with his stepsister, in fact if i remember right she got pregnant but miscarried, Form of inbreeding.

ha..that is gross but just dont say anything just make a call to her parents but dont tell them who you are..believe me it works like a charm
