Can someone explain the formal amendment process?

I'm rather confused, and it's one of the only things I don't really know for an upcoming test. Oh, and how would this question be used as a 'math' essay question as well?

a legislator proposes a new amendment, after the committee approves he bill it needs to be approved by both houses of congress by (at least) two thirds and get ratified by (at least) three quarters of the states.
