It can be settled through a judgment by the court or through an out of court negotiated settlement agreement. You will have to look at state statutes and common (case) law to determine the exact proof needed. Basically, you need to show that whoever is responsible for the street pole box (city, state, county, ect.) was negligent in some way. The mere fact an injury occurred is not sufficient to establish negligence.
You should probably consult a personal injury attorney. They are everywhere - just look for the biggest ads in the phonebook - and most will work on a contingency basis (they don't get paid unless you win the case).

As far as proof, I would first find out of there are any city cameras in the area proving it didn't have a cover.
I would then talk to any nearby businesses or residence in case anyone noticed it was unprotected.
Also, if no one saw the accident, it is harder to prove but not impossible... you have to sue the city.
