Pan's Labyrinth. Not incredibly gruesome, but it has it's moments, and it is an amazing film.
Shaun of the Dead is definitely up there

Does it count if there was 1 part of the movie tat was incredibly gruesome?
At the begginning of "flags of our fathers" it's in this really dark warzone and there's this guy that got shot in the stomach (something like that) and there's this sticky red stuff oozing out. He's all pale and he's gasping. A soldier tries to save him, but then you see this white light and it's just the memory of an old veteran. The rest of the movie isn't too gruesome, and it's an excellent movie.
then again, I hardly ever see gruesome movies. danged parents.
