Dictator Idi Amin was fond of granting himself undeserved medals, awards, and grandiose titles. For example, after declaring himself President of Uganda, Amin insisted his full title be, "His Excellency President for Life, Field Marshal Al Hadji Doctor Idi Amin, VC, DSO, MC, Lord of All the Beasts of the Earth and Fishes of the Sea, and Conqueror of the British Empire in Africa in General and Uganda in Particular."

In what is perhaps his most ludicrous act, he actually proclaimed himself the "King of Scotland." I'm certain the Scots took issue with their new monarch.

Maybe the title has a double meaning: "In an incredible twist of fate, a Scottish doctor on a Ugandan medical mission becomes irreversibly entangled with one of the world's most barbaric figures: Idi Amin. Impressed by Dr. Garrigan's brazen attitude in a moment of crisis, the newly self-appointed Ugandan President Amin hand picks him as his personal physician and closest confidante. Though Garrigan is at first flattered and fascinated by his new position, he soon awakens to Amin's savagery -- and his own complicity in it. Horror and betrayal ensue as Garrigan tries to right his wrongs and get out of Uganda alive." The title may also refer to the fact that the doctor will not let Amin rule him (on a personal level) any longer, i.e., Amin is the last king (or individual) to control Garrigan's actions from then on.

Idi Amin believed he was descended from the Stuart line, which is the name of the Scottish Royal Family.

Well... In South Park, Chef's parents come from Scotland, and his father looks like Forest Whitaker. Just a guess.
