Congrats!! With 3 kids I'm sure that you will be great with the new little one :0) Here are a few things that I could not have lived without during the pregnancy with my three girls:
I agree with everyone about the diaper genie they are more trouble than they are worth, unless you have the newer model those are all right. If not take it back and get a diaper champ!
Bundle Me (amazing for cold nasty days and it comes in a variety of colors)

Baby Bjorn (must have if you don't have one already, especially with 3 other ones! leaves your hands free for other things)

Cup Holder for stroller (MUST HAVE!)

If you're in the market for a new stroller and can afford it go for the bugaboo! It sounds as though your other kids are too old for a stroller and this will be perfect for just one :0) They are amazing strollers and I wish they made doubles!

Diaper Bag:

**This is the best! Even dads like to carry them, they are light and colorful and attach easily to the stroller so you have the basket underneath to store other things!
Congrats!

i think it's all about the same as it was before. the strollers with the infant car seat that snaps right on is a MUST have, and the sling to carry the baby in a pouch if you are out and about is good. but these are good if you can afford them, if not, you do what you did before. If i can suggest one thing to put your pennies towards, it would be an angel care monitor. It's about $100 bucks, but well worth it. the one part goes in the crib, and the other monitor part can go anywhere with you beacause it's battery operated. If the baby stops breathing, (SIDS), the alarm goes off and startles the baby to breath, and you to come running. My friends little guy stopped breathing twice, and the alarm went off. congratulations :) My little guy is 6 weeks.
Oh, and if you havent used that diaper genie and still have the receipt, I would bring it back. It stinks, and it's more hassle than convenience. I bought one and ended up buying a garbage can for the room, way easier.

Congrats!! I think the basics are still in. I love the bassinet I had for my second daughter and I dont know where I would have been if I didnt have it!! It had lights, music and if you turned the wheels in you could rock it!! I loved being able to push her around the house where ever I was to keep an eye on her. I am also a big fan of the exersaucers. My girls both loved them and will play in them for awhile so I can get some housework done. Also the floor toys are great, the ones with the toy bar over them and a mat they can lay on. I hope this helps and congratulations on the new baby!!

The diaper genie is more trouble than it's worth! You have to change the bag at least every day, and it's a huge pain to put the bags in it. I stopped using mine after a couple of weeks and bought a trash can with a lid. It worked much better for me.
Congrats on the new baby!

~A bouncy seat is a must have (the vibrating kind is great), ~electric swing so you don't have to cranck it up all the time,
~a baby gym (little toy that the baby can lay under) is nice when they get out of the sleep all the time stage.
~Another new (well kind of new) thing is a cover up that slips right over the infant car seat, great for wind and bad weather and can't get kicked off like a blanket
These are just a few things I really used a lot and would never go without! Good luck with the new little one :o)

Someone already suggested this but I think the baby slings are awsome I use mine everyday and my son just loves it. Other things that have made my life so easy are a miracle blanket for swaddeling available online here http://www.babyslumber.com/miracle-blanket.html And a white noise machine in the babies room (we use a hepa filter instead of a white noise machine, it works wonderfully and cleans the air at the same time!) Combining swaddeling and white noise helped our little guy sleep through the night before he was a month old! Also if you are planning to breastfed an Avent breast pump is an essential! Congrats on your new little bundle of joy!

First of all........take the diaper genie back. waste of money..........
I reccomend:
A wipe warmer (those things are cold!)
A bottle warmer (never microwave them)
A food grinder (for when baby is a bit older)
An Ocean Wonders Aquarium for the crib......WAY better than a mobile. They can see it while laying on the side of their head, it is appropriate for all ages, the music and lights are very soothing, and it really helps. Mine is 11 months old and still loves it. it is a life-saver for the various stages when the baby does not want to go to bed.
Flannel diaper lap pad things......not sure what they are really called, but they are wonderful to have.
Onesie extenders.....make those really cute clothes fit longer.
Saline nose mist....in case the baby gets congested. It is safe and effective......the only non medicine thing you can give babies to help they breath easier if they are congested.
Infant Tylenol.....trust me, by the time you actually need this, you will want to already have it in the house......ditto with baby gas drops....wal-mart brand are fine.
One good baby Einstein DVD......I recommend Mozart or Beethoven. These will stop a screaming baby mid wail. Mine started watching hers when she was around 2 months. she LOVES them. We don't use them much, but they are essential for quite time or to take a shower.

There are many wonderful answers here. But, I might have the hippest new mom tip for you: flushable eco-friendly diapers! It's a 21st Century hybrid of cloth vs. disposable (forget Genies!), plus adorable patterns and good for the planet. Congrats and have a great birth.

I am sure you will do just fine with all the knowledge you have!! My goodness with 3 kids already you should be telling us what to do. Now, you need to sit back, try to relax and remember you have done this before. After all what children need the most is love and caring and I am sure you and the other children have more than enough of that to share. Good luck Mom!!! And congrats!!!

hi, i have a 3 month old son, and i have a portable swing for him. HE (I) LOVES IT. he loves to sit and swing and listen to the music and look at the lights. its by fisher price and called aquarium take along.
it folds up and will fit in the car, trunk, under a bed, behind the couch or chair. i put it on my bar in the kitchen so that my 1 year old cant get a hold of him. (the swing originally belonged to him).
look it up on fisher prices web-site. it's not too expensive. i got it as a gift, but i also saw it in sears a while back for like 40 or 50 dollars.
GOOD LUCK!!!
