There is no BLOOD relationship with Bhisma Pitamaha and Pandavas or Kauravas. Bhishma was an unmarried old man supposed to be in the position of Grandfather to Pandavas. So they obed him and gave respect and called Pitamaha (Grand father)..

bheeshma pitahmah was the step-brother of pandavas grandfather, that is why he is known as "pitamah".

Bhishma was the youngest son of Shantanu by his first wife Ganga (the holy River). Shantanu was the third king after Dushyanta and Bharata, though the vansh (family or progeny) is said to have started with Bharata the great. Bhishma learnt political science from Brihaspati, the guru of the Devas, Vedas and Vedangas from rishi Vasishta, and archery from Parashurama, also known as Bhargava. He was known as 'Bhishma Pitamaha' (i.e., Bhishma, the grandfather or grandsire) among the Pandavas and the Kauravas. Bhishma is considered to be one of the greatest examples of a dutibound officer.

If Bhishma had married, the Pandavas and Kauravas would not have been born. Bhishma's father, Santanu, remarried after Bhishma's mother died. The new wife's father wanted his daughter's son to be King, after Santanu. So, Bhishma promised that he would not ask to be king and he would not marry so h is kids would not want to be King either. But, it turned out that Santanu's new wife could not have kids. Bhishma wd not break his promise. So there was nobody who would be king. So, a princess and her maidservant were given to a holy man to make them both pregnant. When the princess saw the holy man she got frightened and so she closed her eyes. Her son, Dhritarashtra, therefore was born blind. A blind man cannot be king. The maidservant did not close her eyes, but she went pale and trembled in fear. So her son, Pandu, was born pale and sick, but not blind. So, though Pandu was younger, he became king. When Pandu died, the oldest son of Dhritrashtra (Duryodhana) said he should be King, as his father was oldest. But Kingship was hereditary. So the Kauravas (of the line of Kurus -- Dhritrashtra's family) fought the Pandavas. Bhishma fought on the side of the Kauravas but he was not related either to the Kauravas or to the Pandavas (sons of Pandu).

Bhismaa's brother was Vichitraviirya. Vichithravirya had 2 sons Dhritharashtra and Pandu. Pandu sons were pandavas and dhitharashtraas sons were kauravaas. Bhishma was the grandfather(pithamaha means grandfather in Sanskrit) of Paandavas and Kauravaas

Bhishma Pitamah is the paternal grandfather of Pandavas. Bhisham's step brothers son was Pandu who married Kunti and Madhuri. Kunti was blessed with mystic powers and thus she created four sons i.e. Karan whom she deserted as she was not married at that time when she invoked Sun power and Karan was born. Then she invoked her mystic powers and Yudhishtra, Bheem and Arjuna were born. Nakul and Sehdev are sons of Madhuri and Pandu. As per Indian culture the brother( step or real) of grandfather is also treated as grandfather. Thus Bhisham Pitamah was the grandfather of Pandavas..
