Yes, If it has a sink, or plumbing, and it is under a certain amount of square footage, it has to have some sort or ventilation.

Not in the electrical code but in the mechanical codes..if it is a half bath-sink and toilet if you do not have a window you need to ventilate the bath with a bath/exhaust fan-most are easily installed to 115 volt circuit and the vent pipe ran to the outside.

No, but it is a building code to have one in a bathroom with a tub, shower, or sink if it doesn't have a window. The electrical code pertains only to the manufacture of the fan and how it is wired.
