does anyone know how can health and social care practices and procedures in a specific health and social care setting might be used to improve services to adults

put hilliary clinton in charge and by god something will get done. she is a true believer. god help us seniors and disabled. got one vote waiting for the senator from ny and first lady from bill.

Routine specific assessments by a nurse and social worker to continue to identify needs . To provide referrals, health care, equipment as needed to promote quality of life. There must be specific forms assessing each area of care mental, physical, social, coping ability etc.

Adults need to be proactive in their own care.
They must have the knowledge about the services available in their area, since this will vary from city to city, state/province, country, etc.
If they are unable to advocate for themselves they need someone who can, and to make sure they are applying for all the support they need and are entitled to.
