The Shining.

Most scary movies just gross you out or have people jump out and scare you. The Shining doesn't do either. It's incredibly creepy and disturbing as you watch the Hotel ghosts slowly drive Jack mad.

The most creepiest movie that had me really spooked at times was The Grudge. It's not a horror movie with blood and murders but it was really creepy. The Japanese have a way of enhancing fear and they did it perfectly in this movie.
Another one I would say The Ring (Japanese version) not the Hollywood production.

I agree with the first answer. Alien was terrifying when it first came out. There were things in it that had never been done before - the gruesome robot's talking head, the digusting monster dripping snot, the little beastie that comes bursting out the guy's belly, the crablike thing that attaches to the guy's head. Ridley Scott rules.
