About as good as getting pregnant from ordinary semen. In other words, if you don't want to risk pregnancy, don't come into contacy with ejaculate or pre-ejaculate.
You also can catch any STD from the pre-ejaculate. Protection needs to be applied as soon as the penis is erect.
