An E111 form has been replaced by the one shown below but you will note it is only valid for the E.U. the Canary Islands like Gibralter are a tax free state and therefore not in the E.U.
The European Health Insurance Card (EHIC) is the replacement for the E111. As of 1 January 2006, E111s are not valid.
Persons who are normally resident in the United Kingdom (UK) are entitled to a UK issued EHIC.
The EHIC can be used to cover any necessary medical treatment due to either an accident or illness within the European Economic Area (EEA). The card entitles the holder to state-provided medical treatment within the country they are visiting and the service provided will be the same as received by a person covered by the country's 'insured' medical scheme. This may not cover all of the services you would expect within the UK and you may have to make a contribution towards the care you receive
The EHIC is valid within the EEA, which consists of the European Union (EU) plus Iceland, Liechtenstein and Norway. Switzerland also operates the same arrangement.
For helpful and convenient information on how to obtain a European Health Insurance Card in the United Kingdom you can telephone EHIC Information on:
Calls cost £1.50 per call from BT Landlines. Calls from mobiles and some networks may be higher and you must have the bill payer's permission.

www.dh.gov.uk - The E111 no longer exists. You need the new EHIC (European Health Insurance Card) which you can apply for now - free - online at the above website.
You will also need some travel insurance as the EHIC only covers emergency treatment NOT air ambulance home or serious surgery.
Happy holiday!

They now issue you with a European Health Card which lasts quite a few years. So if you travel to European countries its worth getting one anyway. You can use it in the Canary Islands. Make sure you get travel insurance too - don't use the travel agents its a rip off, get it from the post office etc.

You do not have to have a European Health Card (E-111 are finished now) to visit anywhere, but it is very advisable in case you need medical help.
