The appraisals that I have done and received in the past are looking at several aspects of how well a person knows, applies and follows thru their duties. Individual thinking, going beyond the call of duty, customer service, attendance, getting along with coworkers, are all important things to look at. Depending on the time frame between appraisals, monthly, quaarterly, semi, or yearly, keep a journal and jot down good and bad points, discuss these items with the person between appraisals and look for improvement or consistancy in performance. Always, always, always, give an action plan and time frame for performance improvement and let a person know their value to the department. Do not let personal feelings come into play or come thru the appraisal and do not attack a person due to personality conflicts. Neutrality is key. Good luck....

I suggest two things: a) a comparison of set goals -usually one year ago- and the results obtained. If there were no goals setting exercises, then the expected outcome of the role might work, and b) a 360 evaluation, this is from peers, subordinates and people in the level above the person who's been evaluated -if you can include outside the company customers would be optimum-. In the later, make sure that you ask for specific, actionable feedback (not stuff like : he's a nice person).

However, for senior executive there can certain more information, such as 'Key Tasks Achieved' in given time, etc.
