No, a file is fragmented when it is written to differnet parts of the disk. The system keeps track of all the small parts so you never notice. It does make it a bit slower to get the entire file though.

No... (and I'm not sure why you would think that)....

If you want to stay low on fragmentation, defrag your hard drive regularly (or as often as you can), and try to keep a reasonable amount of space free on your hard drive. I have 60 GB of space, and I usually have 5 to 10 GB free.

I have a software called "Diskeeper Professional" which I set to automatically defrag my hard drive in the background, 24/7. I never have to formally defrag my hard drive, since Diskeeper Professional is always working on it.
