I suspect that you have typed the question incorrectly and that brackets are required as follows:-

2v/(v² - 16) + v/(v - 4)

= 2v / (v - 4).(v + 4) + v / (v - 4)
= (2v² - 8v + v² + 4v) / (v - 4).(v + 4)
= (3v² - 4v) / (v - 4).(v + 4)
= v.(3v - 4) / (v - 4).(v + 4)

I'm not sure but my answer is:
-15v^3+66v^2-8v
