Should I still put that company's contact information? Or just his contact information?

He's currently in-between jobs, so would it be okay to just list his personal number and email address?

I still use the name of an ex co-worker as a business referee and he is also between jobs at the moment. As far as I'm concerned, you have worked with that person, even if it was in the past and they know how you perform, so I believe it to be perfectly reasonable to use them as a referee

Yes, listing his personal number and email address would be the best option if he's still willing to be listed as a reference for you. You might also add in a note, after the phone number and email, that he worked with you at the company from [date] to [date].

I feel that you should ask him as to how he wishes to be listed on your reference list. Just do as per his preference.
