I don't have a clue on how many times it has happened, but my favourite was when the Thrashers did it against the Flyers last season, Nov 18 2005. They were down 5-3 late, and then scored two goals in the final two minutes with six, yes six, forwards out on the ice. Kovalchuk, Hossa, Kozlov, Savard and Mellanby were on for both, while the two goal scorers Bondra and Stefan were on for one each. Stefan scored the tying goal on a rush with 7 seconds to go.
Kovalchuk managed to assist both of those goals AND stop a Gagne empty net breakaway all in those last 2 mins.
Kovalchuk then sprung de Vries for a breakaway to win the game in OT, giving Kovalchuk 2g and 3a on the night. It was simply an incredible game, and certainly the only time I've ever seen a team score two goals in the final two minutes with six forwards on the ice to tie a game.

Yeah there have been a great sort of video games this year the place that resulted, it fairly is not as un-user-friendly as you think of, i think a Montreal interest, the Canadiens pulled the goalie and Markov scored with a million.9 seconds left, and then they won in time beyond regulation.

I don't think anyone has the answer to that. No one keeps track of when teams pull their goalie.

I've seen a few times where that team wins and more often ties for the point, but it's rare. no one tracks this stat as far as I know.
