Well, I think that eating disorders are caused by a combonation of things; a distorted self-image or a desire for a sense of control in a girl's (or boy's) life. I know people who are bullimic/anorexic because their life is crazy, and they get a sense of control by being able to decide if they're going to eat, or not. It can also be a cry for help. There are teens who are so desperate for someone, anyone, to acknowledge their existence, that they'll resort to harming themselves (ie: cutting/eating disorders). And for others, yes, an eating disorder can result from low self-esteem and a distorted body image. Only, if you look at the motive for why most women want to be thinner, it is because they want attention, and affection - be it from guys, or from their parents, or whomever.

Eating disorders can be caused by many different factors in a person's life ... low self image is only one of them, and it is not the 'main cause' ... which is a 'visualization' dysfunction, where the people can't see their 'real body' but see someone who is 'fatter' or 'thinner' than they truly are.

I believe so. Some of the people are beautiful and don't realize it. Then when they get the eating disorder, they do look bad.
