Hitler was trying to get the rich & poor to buy his Master Race idea, he had to appear to everyone as being a humble common man with great ideas for the masses. By including everyone into the Master Race idea he had to appear as one of them; except to those closest to him. They saw his real face but if it brought them honor, glory & wealth, they didn't care as long as he took them along!
His speach writers were inspired but it was his delivery that carried the masses. He was modeling his empire after the Roman Empire. The Roman Empire rose from the ashes 3 times; thus the name 3rd Reich; matching German's 3rd rising from ashes with the Roman Empire(Franko/Prussian war titled Reich) 1st rising. WW1, 2nd rising, WW2 3rd.

It's just a matter of names, he prefered to call himself "Führer" which means "leader", and in the Nazi propaganda he had a much higher position than what an emperor would be. He was an almost religious figure in this propaganda and he had absolute power, similar to Stalin in the USSR. Yes, "Reich" means "empire", I wonder why the German word is always used in English. The difference to a monarchy was that he did not descent from any bloodline of rulers and that he didn't get children to continue such a bloodline. As I wrote before, in the way he was almost worshipped he was more than an emperor. Germany was a monarchy with emperors (and that was also called Reich, it was the Second Reich) until 1918, and there was never such a personality cult for these emperors as it was for Hitler.

It would have been counterproductive for Hitler to call himself a king, because after all, he had the keep the facade of equality and compassion visible to the German people.
Who knows what might have happened had he won the war.
But really, Reich Chancellor is just as good as king if you had the power Hitler did. A dictator by any other name is still a dictator.
This is a had question to answer, and I have no real insight on this other than speculation.

Well if it's a monarchy, there needs to be some one to continue the line, such as a son, daughter, brother or nephew. Hitler had none, and it seemed like he had no interest in women in that way. That's why he continued with the dictatorship so he could name pretty much anyone as his successor.
