It depends on what type of service you work for and where in the country you work. Bigger cities tend to pay more, the pay tends to be higher on the East and West coast, and less in the middle and Southern states. Private services tend to pay less (AMR, Acadian, Rural/Metro, etc), while firefighter/paramedics as part of a fire department tend to get paid more. Hospital based and municipality based services usually fall somewhere in the middle.

Generally, starting salary tends to be around 10-12 a hour w/usually some mandatory overtime a week. The max salary tends to be 16- 18 an hour, and it can take ten years to earn that much.

Some jobs pay a lot more. Flight medics, on helicopters or fixed wing, usually start out at 18 an hour or so, but these are some of the most sought after jobs in EMS, with about 200 applications for every open position. Oil rig jobs also pay well, and the pay ranges from 250-450 a day, with the usual schedule being 14 days on and 14 days off.
