The Walter Drake catalog (sorry, you'll need to look them up in Google or Dogpile, as I don't know their web address) has a lot of personalized items. I think I have actually seen these in there.

I got mine at a local fabric store, though. They had labels with a lot of the more usual names pre-made and ready for sale. There was an order blank to use if your name wasn't available. Phone your local fabric shop to see if they have these. I got mine over 20 years ago, unfortunately, so I have no idea if they're still even common.
